Notes:
- The 12-month Membership fee is due in full upon joining.
- Payments are due as services are rendered.
- No other discounts can be combined with membership.
- It is the sole responsibility of the member to schedule their exams & cleaning by arranging all the appropriate appointments within the 12-month membership period. If the appointments are not used, the member will not be entitled to a refund.
- There is a cancellation fee for any appointments that are failed or late canceled (less than 48 hours)
Exclusions:
- This membership is NOT an insurance plan, and can only be used at Advanced Dental Care & Aesthetics
- This membership is nonยญ transferrable.
- Membership discount cannot be combined with dental insurance or any other discounts.
- Discounts cannot be applied to treatment completed before membership.
